OUR SERVICES

Direct Primary Care and Functional Medicine

Patients may choose to see Dr. Mann for Direct Primary Care or Dr. Reader for Functional Medicine and are not required to see both.  Patients may also choose to see both of our physicians at a discounted price.  Direct Primary Care services are offered as a monthly membership while Functional Medicine services are offered in a package format.  Unfortunately, we are not able to enroll patients who are currently enrolled in Medicaid or Medicare at this time.

DIRECT PRIMARY CARE

  • Adults (Age 18 - 64)              $150/mo​

  • There is a one time enrollment fee of $100 per household.

  • Lab, specialist, radiology and pharmaceutical costs not included.

FUNCTIONAL MEDICINE

  • $1800 Five visit Wellness Package.  Includes an initial consultation (up to 90 minutes) + four follow-up visits (up to 60 minutes each) + time between visits reviewing questionnaires, laboratory results and formulating a personalized treatment plan.​

  • $400 Individual Functional Medicine visits. 

  • $400 Functional Medicine Records Review and Consultation.  Includes review of previous labs and records pertinent to Functional Medicine + review of Functional Medicine questionnaires + one consultation (up to 60 minutes).  Does not include specific treatment recommendations. 

  • Laboratory, equipment, supplement and pharmaceutical costs not included.

  • A complimentary "Meet and Greet" via phone or in-person is offered prior to commitment.
